Controversial opinion: all penalties should be reviewable on a challenge system.
That said, review procedures need to change. Refs should have a time limit for their review. 2 minutes I think would be fair. If they can't agree to change the call after all the footage they view in that time, then the one made on the ice stands.
That would catch all the clear and obvious stuff and still keep the game flowing reasonably well without sacrificing either too much.

4 officials were on the ice, and not one of them caught the actual incident in the real sense.
