Truth is, Jones don't think he's worthy cause he hasn't proven himself. Only reason he has the Interim belt is due to the lack of high caliber heavyweight contenders.
I don't believe Jon fears Tom man to man. Like in a street fight to the death, Jon has no fear of any man. That being said, Jon knows that in a cage fight, Tom is the biggest threat out there right now and the fear Jon has is about his legacy and possibly taking his first L. That's a very real fear.
